This page is your one stop shop for submitting an event request. You will need to be signed into your MillCity Church Microsoft 365 account to complete each form.
Step One
Begin by completing the G.R.A.C.E. Action Plan. This should be completed at least 90 days before the event. This is a high-level questionnaire that defines the who, what, where, when, how, and (most importantly) the why of the event.
Step Two
Complete the corresponding form for each team needed for your event.
Request an event's webpage, print, video, marketing, and more. This form may take up to 30 minutes to complete, depending on the amount of project requests. Please note that different types of projects have varying lead times based on scale and complexity.
Catering
Please review this event's corresponding seasonal menu before placing completing the catering form. This form must be completed at least three weeks prior to event.

MillCity Kids
If more than 25 kids, must be completed at least six weeks prior to event. If less than 25 kids, must be completed at least three weeks prior to event.
